What You'll Do:
  • Evaluate patients and develop individualized therapy plans
  • Provide in-home physical therapy treatments
  • Improve mobility, strength, balance, and functional independence
  • Educate patients and caregivers on exercises and safety techniques
  • Monitor progress and adjust treatment plans as needed
  • Complete accurate and timely documentation
What We're Looking For:
  • Active Texas Physical Therapist (PT) license (required)
  • Home health experience preferred
  • Strong clinical judgment and patient care skills
  • Ability to work independently in a home setting
  • Excellent communication and documentation skills
  • Reliable transportation & valid driver's license
  • Kinnser/WellSky EMR experience (preferred)
