In July of 2020 UW Health Care Direct (formerly known as Chartwell Midwest Wisconsin) officially consolidated all community- based home health services to Chartwell from UW Health and Unity Point Health leading to the establishment of UW Health Care Direct. UW Health Care Direct is dedicated to providing home health care services in the Dane County area, ensuring the highest quality of care for the community.

POSITION SUMMARY - This position is part time (24 hours a week)
Responsible for the referral intake process including collection of patient data, processing and coordination of services for patients with nursing, pharmacy, operations, and reimbursement staff as well as external hospital, clinic, physician, and home health providers.
ESSENTIAL JOB KNOWLEDGE
  • Manages patient referral list, including accepting new referrals, initiating onboarding process and communication with appropriate team members.
  • Assists infusion nurses with patient intake.
  • Monitors patient progress toward admission to service collaborating with providers, referral sources, community agencies, and organization personnel to ensure safe plan of care and establish mutual goals.
  • Documents in electronic medical record per organizational policies and procedures.
  • Confers with stakeholders (physicians, patient/family, health care personnel, etc.) during the onboarding process to establish mutual goals.
  • Assists the Infusion Nurses with monitoring the status of patients readmitted to hospital and documents appropriately.
  • Participates in the evaluation, problem identification, planning, and implementation functions for patients who are referred to, or who may be referred to UW Health Care Direct.
  • Communicates effectively with referral sources and patients regarding patient financial responsibilities and insurance coverage for home care services.
  • Communicates patient information (including but not limited to insurance and authorization status) to team members in a timely manner.
  • Serves as a resource/consultant to on-service patients, home health agencies, and other external stakeholders when coordinating discharge from a healthcare facility.
  • Assists infusion nurse staff with operational tasks as delegated and appropriate.
  • Acts as a preceptor to new staff.
  • Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
  • Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
  • Quality: Focuses on improving processes and champions efficiency in daily work activities.
  • Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization's occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM023 and Mission Statement).
  • Takes on special projects by the supervisor and performs other duties as assigned.
JOB REQUIREMENTS
Licenses:
  • Valid driver's license in the state of Wisconsin
  • State of Wisconsin professional licensure as a Registered Nurse
Education:
  • Minimum of Associate's degree of nursing (required)
  • Bachelor's degree of nursing (preferred)
Experience:
  • Minimum of 2 years of recent nursing experience.
  • Discharge planning, care coordination, infusion, or home health experience preferred.
Miscellaneous:
  • CPR/BLS certification required.
  • Excellent organizational, interpersonal, communication, and customer service skills required.
