Clinical Pharmacist

The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry‑level position introduces the practice of clinical pharmacy in the alternate setting and involves coordinating patient services with the nursing department, monitoring patient progress toward goals, and educating patients, caregivers, staff, and other professionals regarding appropriate medication use.

Responsibilities
  • Determine the suitability of individual patients for home care.
  • Collect and organize all patient‑specific information needed to assess suitability.
  • Assess patients’ suitability for home care in accordance with Option Care policies.
  • Evaluate medical condition and prescribed medication therapy for suitability and establish clearly defined outcome goals.
  • Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans for pharmacotherapy of home care patients.
  • Make suitable recommendations for modifications in prescribed medication therapy or monitoring parameters when appropriate.
  • Collect and organize patient‑specific information to prevent, detect, and resolve medication‑related problems and make appropriate medication therapy recommendations.
  • Identify appropriate monitoring parameters for each patient.
  • Complete nutritional assessment of patients and make appropriate recommendations.
  • Modify pharmacy care plans based on evaluation of monitoring data and other pertinent information.
  • Apply pharmacokinetic principles and formulas competently.
  • Assess patient laboratory values competently.
  • Provide medication‑use education to Option Care staff, other professionals, home care patients, and/or their caregivers.
  • Use effective patient education techniques for counseling of patients and caregivers.
  • Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings.
  • Communicate pertinent patient information to and from the home and other settings using a systematic procedure.
  • Prepare and dispense medications following Option Care policies, accurate equipment calibration, and appropriate techniques.
  • Prepare medications with correct concentration, stability, compatibility, and appropriate storage.
  • Adhere to safety and quality assurance practices.
  • Prepare labels conforming to organization policies and include all necessary ancillary labels.
  • Inspect final medication before dispensing and ensure authorization to dispense.
  • Comply with state/federal laws, USP standards, DEA regulations, and perpetual inventory system policies for schedule II narcotics and controlled substances.
  • Manage medication administration equipment and related equipment, including programming, maintenance, and troubleshooting.
  • Participate in assessing, managing, and reporting medication‑related incidents and equipment errors or failures.
  • Complete QAR documents according to Option Care policy and monitor/report discrepancies in perpetual inventory systems for controlled substances.
  • Provide concise, applicable, and timely responses to drug information requests from healthcare providers and patients, based on literature analysis.
  • Attend educational seminars or other CEU opportunities to promote professional competency and comply with state‑mandated CEU requirements.
  • Manage the use of investigational drug products according to regulations and Option Care policies.
  • Utilize pharmacy support personnel effectively and prioritize workload while ensuring accurate work by support staff.
  • Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines.
Basic Education and Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active pharmacy license as granted by the State Board of Pharmacy.
  • One of the following: (a) at least one year of experience in sterile IV admixture procedures; (b) knowledge of non‑sterile compounding, immunizations, and medication therapy management (MTM); (c) completion of an ASHP accredited Pharmacy Residency Program; (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; or (e) equivalent experience approved by the VP of Pharmacy or SVP of Clinical Services.
Basic Qualifications
  • Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction.
  • Basic level skill in Microsoft Word (document editing, formatting, and basic functions).
  • Basic level skill in Microsoft Excel (data entry, formatting, and basic functions).
  • Basic level skill in Microsoft PowerPoint (slide creation, formatting, and presentation).

Preferred Qualifications & Interests (PQIs)
  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Experience in home care, hospital, or other clean room settings.
  • At least one year of indirect management experience of team members, including training, assigning work, and performance evaluation.

Salary

Pay range: $97,113.10 - $161,862.67. Salary determined by education, experience, knowledge, skills, and abilities, as well as internal equity and market data.
