In this role, you will be responsible for a variety of tasks tailored to meet the unique needs of hospice patients:

• Conduct comprehensive clinical assessments of patients facing terminal illnesses, developing personalized treatment plans that address emotional, social, and psychological needs using evidence-based practices.
• Utilize therapeutic interventions, including cognitive-behavioral therapy (CBT) and solution-focused therapy, to aid patients in coping with their diagnoses and enhance their overall well-being.
• Maintain precise and confidential clinical records in compliance with HIPAA regulations, ensuring that patient information remains secure.
• Engage in multidisciplinary team meetings to discuss patient care strategies, share insights, and coordinate services that align with the patients' treatment goals.
• Provide education and support to patients and their families regarding coping mechanisms, palliative care options, and available community resources, empowering them during challenging times.
• Participate in ongoing training and professional development opportunities, staying informed about best practices and innovations in social work and hospice care.

Your primary focus will be on patients in a hospice setting, typically those experiencing life-limiting illnesses. This role demands a high level of sensitivity and understanding as you will be working with individuals and families navigating end-of-life issues, ensuring that their emotional and psychological needs are met compassionately and effectively.

• A master's degree in Social Work from an accredited institution, illustrating a solid academic foundation in social work practices.
• A valid state license to practice as a Licensed Clinical Social Worker (LCSW), demonstrating your professional credentials.
• At least 3 years of clinical experience providing psychotherapy, particularly with terminally ill patients or within a hospice environment.
• Experience with various psychotherapy interventions, equipping you to customize your approach for individual patient needs.
• Strong communication and interpersonal skills, essential for building rapport with patients and collaborating with multidisciplinary team members.

• Experience in a hospice or palliative care setting, providing direct support to end-of-life patients and their families.
• Proficiency in documentation and case management software to manage patient information efficiently.

The anticipated salary for this role ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
