Job description

Admissions Registered Nurse
The Admissions Registered Nurse is responsible for making the initial visit with patients and families to explain and initiate services. This role focuses on completing a comprehensive assessment of the patient, environment, and support system and admitting to the appropriate program. The Admissions Nurse develops an initial plan of treatment, completes necessary documentation, and collaborates with the interdisciplinary team to meet the immediate needs of patients and families.
***The role is primarily centered within the Green Bay and Sturgeon Bay territories, but may travel anywhere within our service area for admissions. ***
Schedule: This position is scheduled for 36 hours per week, working 12:00 p.m. - 9:00 p.m. The role includes every 4th weekend rotation, approximately 10-12 call-back shifts per year, and 1-2 holidays annually.
Key Job Duties:
  1. Review information concerning the patient's medical background
  2. Identify the appropriate decision maker/agent and follow through with Durable Power of Attorney for Healthcare (DPOAHC), activation and "In Lieu Of" forms
  3. Provide an explanation of hospice and palliative programs and answer questions appropriately
  4. Review and complete admission forms with the patient/family
  5. Complete a comprehensive nursing assessment which includes medication reconciliation and obtain orders as needed
  6. Collaborate with members of the medical team to determine appropriate services as needed
  7. Notify referral sources and attending physicians of admission to the hospice or palliative program
  8. Assess need for and order supplies and durable medical equipment
  9. Enter appropriate information into the electronic medical record and initiate follow up
  10. Communicate with the primary team and support services to meet the needs of the patient
  11. Collaborate with the Director of Patient Services and other departments as appropriate on process improvement efforts
  12. Participate in focus groups or other specialized work groups

Qualifications
  • Current unrestricted registered nurse licensure from the State of Wisconsin
  • At least one year of clinical experience as a registered nurse
  • Excellent critical thinking, interpersonal relationship building, and organizational skills
  • Valid Wisconsin Driver's License, reliable transportation and proof of current automobile insurance coverage
