RN Unit Manager

Location: Milford, Delaware

Company: Polaris Healthcare and Rehabilitation Center

Shift Type: Full-Time, Monday - Friday

Job Summary: Polaris Healthcare and Rehabilitation Center, a beautiful 100-bed facility located at the Milford Wellness Village, is seeking a compassionate and experienced RN Unit Manager to join our team. As a RN Unit Manager, you will be responsible for managing the unit, utilizing the nursing process to assess, plan, implement and evaluate resident care, and ensuring that our residents receive high-quality, compassionate care.

Responsibilities:

  • Utilize the nursing process to assess, plan, implement, and evaluate resident care
  • Assess signs and symptoms indicating changes in the patient's condition
  • Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
  • Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards
  • Revises the plan of care according to evaluation, changes in the medical plan of care, and effective/ineffective nursing interventions
  • Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor, including establishing priorities

Qualifications:

  • Must be a licensed RN with a valid license
  • Familiarity with Federal, State, and local long-term care requirements
  • Two years of professional nursing experience
