Case Management 2 years exp required

FLU VACCINE IS REQUIRED at UMMC

Only medical exemption is allowed and requires documented evidence from a health care provider of a past severe reaction to the vaccine.

FINGERPRINT REQUIREMENT

Remote process must allow 10 days.
In-person can be done at Jackson or Grenada campus the week prior to start.

HIRING REQUIREMENTS (HIGHLY EXPERIENCED ONLY)

This hiring manager is looking for highly experienced candidates with acute inpatient case management and discharge planning experience. Training on unit will be minimal - candidates must be able to pick up quickly and work efficiently.

REQUIRED

  • Minimum 3 years of Acute Inpatient Case Management experience
    • Must include at least 1 year in a LARGE hospital (500+ beds, LEVEL 1 / ACADEMIC medical center)
    • Does NOT include telephonic, utilization management, or therapy experience
  • Strong computer skills, Epic EMR experience
  • Nursing degree preferred (RN, BSN; ADN may be considered)
  • BLS certification through AHA
  • CCM preferred but not required
  • Acute discharge planning experience
  • Recent (within last 2 years) acute inpatient case management experience

SHIFT & SCHEDULE

  • Mondayโ€“Friday: 8:00 AM โ€“ 4:30 PM
  • Weekends: None
  • Holidays: Rotated as needed
  • Case Load: 15โ€“20 patients/consults per day

JOB DETAILS

  • Covers all inpatient units throughout the hospital
  • RNs work with Social Work team (15 full-time, 17 part-time social workers)
  • UMMC has contracts with 5 local facilities with 10 beds reserved at each facility for complex placements
  • Financial assistance limited to uninsured or underinsured patients
  • UMMC is the only Level 1 trauma hospital in the state; receives highly complex cases

  • Scrub Color/Dress Code: Any color scrubs or business professional attire
  • RT for Discharge Planning: Consult as needed
  • Pharmacy for Discharge Planning: Consult as needed
  • Orientation: 2 weeks (includes general facility orientation)
  • Will work alongside CM team; will never be working alone
