Job description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Location: Location: Lafayette, CO
Pay: $22.77-$24.81
Schedule: PRN - SAT/SUN: 6am to 6pm
Working @ IPU and Homecare
JOB SUMMARY:
The Certified Nursing Assistant (CNA) is responsible for providing personal services to assigned patients. Provide direct patient care, collaborates with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various care settings. Provide patient care in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Hospice of Northern Colorado. Upon request, participates in all areas of clinical services.
EDUCATION AND QUALIFICATIONS
  • Must possess a current Colorado C.N.A. license
  • Must have a minimum 6 months experience - hospice or home health experience preferred
  • Must be a high school graduate or possess a GED
  • Must possess CPR certification or be able to obtain within 60 days of hire
  • Must possess a valid driver's license, provide proof of insurance (a copy of your "Declarations Page" indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage) and have reliable transportation
  • Requires travel to multiple work locations
  • Must comply with agency health screening and immunization requirements
  • Must be able to demonstrate computer competency

ESSENTIAL DUTIES
  • Provides patients with personal care as designated in the plan of care; understands Colorado law related to the supervision and delegation process from RN to CNA; understands general physical symptoms as they relate to end of life and has an understanding of non-medical interventions
  • Performs transferring and positioning utilizing appropriate equipment per standards of practice and with patient safety as priority; understands good body mechanics and use of safety and transfer devices to prevent injury to self and patient
  • Incorporates patient/family preferences and goals within the plan of care
  • Supports and respects the patient's and family's unique diversity, including spiritual and cultural beliefs
  • Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards
  • Provides patient and family education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members
  • Works collaboratively with volunteers and provides ongoing support and guidance
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely open manner; engages collaboratively in conflict resolution processes as needed
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external
  • Participates in mandatory educational activities and required meetings
  • Is competent using agency information and technology systems
