Job description

  • JOB SUMMARY
    • As an employee of Trinity Health System it is the responsibility of this and all positions within the hospital to support the mission and role of Trinity Health System. In addition, this position will actively support the Hospital`s Strategic Plan in the areas of People, Service, Quality, Finance, and Growth, in a team based environment. the employee also performs all job duties in compliance with laws and regulations in the observance of accrediting agency standards.Evaluate patients, develop plan of care and provide treatment within the guidelines of physician referrals, participate in efficient, cost effective planning and operation of the Home Health Department.
  • QUALIFICATIONS
    • Education
      • Minimum of Bachelors Degree in Physical Therapy. Masters and/or doctorate preferred.
    • Training and Experience
      • Two (2) years experience in Physical Therapy practice required.
      • Ohio License required. May also be required to obtain a WV license.
      • BLS provider or obtain within 3 months of hire.
    • Health and Background Requirements
      • Employment contingent upon successful completion of:

      • Must be physically able to perform all job duties as assigned.
