Job Summary:

We are seeking a compassionate and skilled Per Diem Home Health OASIS Registered Nurse (R.N.) to join our team of dedicated healthcare professionals. As a Per Diem OASIS R.N., you will be an integral part of our home health team, providing high-quality, patient-centered care to our clients in their homes. This is a per diem position, with flexible scheduling to meet the needs of our clients.

Responsibilities:

  • Conduct comprehensive assessments of patients in their homes, identifying their needs and developing individualized care plans to meet those needs

  • Provide skilled nursing care to patients, including wound care, medication management, and education

  • Collaborate with other healthcare professionals, including therapists, social workers, and home health aides to ensure comprehensive care

  • Maintain accurate and detailed documentation of patient care, including OASIS assessments and progress reports

  • Communicate regularly with patients, families, and healthcare teams to ensure continuity of care and address any concerns
