Job description

Site Physician (Bilingual Spanish Required) – Bronx, NY

Location: Allerton, Bronx, NY
Setting: On-site – Day Treatment Center (DTC)
Schedule: Monday – Friday | 8:00 AM – 5:00 PM (1-hour lunch)
Position Type: Full-Time Temporary with Potential for Temp-to-Perm

About the Opportunity

We are seeking an experienced Site Physician to provide high-quality, patient-centered primary care services within a Day Treatment Center. Working under the direction of the Regional Medical Director, the Site Physician will oversee the clinical care of participants while collaborating with an interdisciplinary healthcare team to deliver comprehensive, coordinated medical services.

This role is ideal for a physician passionate about caring for older adults and medically complex populations in a collaborative outpatient setting.

Must Haves
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • Active, unrestricted New York State medical license
  • Board Certified in Internal Medicine, Family Medicine, or Geriatric Medicine
  • Fluent in Spanish (Required)
  • Active DEA registration
  • Active Medicare and Medicaid provider IDs
  • Experience in Primary Care, Internal Medicine, or Geriatric Medicine
  • Strong clinical assessment, diagnostic, and treatment planning skills
  • Excellent communication and interdisciplinary collaboration skills
  • Experience with electronic medical records (EMR)
Nice to Have
  • Experience working in PACE, Adult Day Health, or Day Treatment Center settings
  • Experience managing medically complex geriatric populations
  • Knowledge of value-based care and chronic disease management
  • Leadership or supervisory experience
  • Experience collaborating with interdisciplinary care teams
Responsibilities
  • Provide comprehensive primary medical care to participants in the Day Treatment Center
  • Perform evaluations, diagnoses, treatment planning, and ongoing medical management
  • Develop individualized care plans in collaboration with the interdisciplinary team
  • Monitor chronic conditions and coordinate specialty referrals as needed
  • Participate in care conferences and interdisciplinary team meetings
  • Maintain timely, accurate, and compliant medical documentation
  • Ensure quality, regulatory, and organizational standards are consistently met
  • Collaborate with nursing, rehabilitation, social work, and behavioral health professionals to optimize participant outcomes
