Float Chaplain - Unity's Full Service Area

The Chaplain provides spiritual care and support to patients and families, honoring their individual beliefs and preferences. This role is an integral part of the interdisciplinary team, ensuring that spiritual needs are thoughtfully addressed throughout the hospice journey.
Schedule: This position is a 0.8 FTE, working 32 hours per week on a schedule of four 8-hour shifts, typically 9:00 a.m. to 5:00 p.m. each day. The workweek consists of four days, with one flexible weekday off based on departmental and patient care needs.
As a Float Chaplain, this role provides coverage for palliative care patients, chaplain paid time off (PTO), and FMLA leaves by assuming patient caseloads as needed. The Float Chaplain also supports interdisciplinary team (IDT) coverage and assists with patient visits when another chaplain's caseload increases to help ensure continuity of care.
The position participates in the on-call rotation approximately one full weekend every 6-7 weeks, providing chaplain coverage for the entire Unity service area. Weekend on-call hours are Friday from 5:00 p.m. to 7:00 p.m., Saturday from 9:00 a.m. to 5:00 p.m., and Sunday from 10:00 a.m. to 4:00 p.m. In addition, the Float Chaplain works one holiday approximately every 1.5 years, with holiday hours of 9:00 a.m. to 5:00 p.m.
Territory: This is a float chaplain position serving Unity's entire service area of 15 counties. Travel throughout Unity Hospice's full service area is required to support patient and family needs.
Key Responsibilities:
  • Provide spiritual counseling and support to patients and families as requested
  • Complete spiritual assessments and develop individualized spiritual care plans
  • Coordinate with patients' clergy and facilitate spiritual services, prayer, or sacraments
  • Assist families with funeral planning when requested
  • Collaborate with Volunteer Services to connect spiritual care volunteers with patients
  • Serve as a liaison to the local religious community and provide education on hospice care
  • Partner with the bereavement team on memorial services and programs
  • Participate in interdisciplinary team (IDT) meetings and contribute to care planning
  • Support process improvement initiatives related to spiritual care services

Requirements
  • Bachelor's degree from an accredited school of theology, religious studies, or related studies and/or certification in hospital or pastoral ministry required. Master's degree in theology, divinity, or pastoral studies preferred.
  • Board Certification by the NACC (National Association of Catholic Chaplains or APC (Association of Professional Chaplains) or working toward certification preferred.
  • Two units of Clinical Pastoral Education are preferred.
  • Experience in hospice and palliative care and an understanding of hospice and palliative philosophy is preferred.
  • Valid Wisconsin Driver's License, reliable transportation and proof of current automobile insurance coverage
