What You’ll Do

  • Conduct intake assessments and submit documentation for prior authorization based on pay sources
  • Develop care plans with nursing diagnoses, physician input, and measurable goals
  • Review medical data regularly to assess service effectiveness and rehabilitation potential
  • Address individual or employee concerns and complaints
  • Ensure aides are properly trained to meet individual needs
  • Perform supervisory visits and provide instruction to individuals, families, and caregivers
  • Coordinate home care services through timely documentation and data transfer
  • Contribute input on agency policies and participate in advisory boards or committees as requested

Qualifications

  • Nursing degree with a valid Missouri RN license
  • Minimum 2 years of relevant nursing experience
  • Strong communication and analytical skills
