RN Case Manager – Travel/Local
Glendale, AZ

We’re hiring an experienced RN Case Manager for a hospital-based role in Glendale, Arizona. This position focuses heavily on discharge planning and coordinating post-acute care to ensure safe, efficient patient transitions.


Position Overview
  • Conduct initial case management assessments

  • Develop and coordinate discharge (DC) plans with patients, families, and care teams

  • Arrange post-acute services based on patient preferences and needs

  • Collaborate with physicians, nurses, and interdisciplinary teams

  • Ensure compliance with hospital policies and regulatory standards

Requirements
