Job Summary:

Area Agency on Aging of Pasco-Pinellas is seeking a compassionate and experienced Care Navigator to support those living with dementia and their caregivers through the CMS GUIDE (Guiding an Improved Dementia Experience) Model. The Care Navigator serves as a key liaison among dementia patients and caregivers to ensure seamless support and improved outcomes through in-home or on-site assessments, care planning, dementia support, and coordinating services.

This role is dedicated to enhancing the quality of life for individuals living with dementia, reducing caregiver strain, and helping patients remain safe in their homes.

Duties and Responsibilities:

A Care Dementia Navigator provides guidance, support, and resources to individuals living with dementia and their caregivers. They help families understand the condition, access services, and navigate healthcare and social care systems. Their role includes offering emotional support, connecting individuals to community resources, and ensuring they receive appropriate care.

• Serve as the care navigation contact for individuals living with dementia and their unpaid caregivers, to provide support.
• Complete beneficiary assessments, home visit assessments, and caregiver assessments with direction and oversight by the Participant organization.
• Maintain ongoing engagement with beneficiaries and caregivers to address evolving needs.
• Participate in ongoing monitoring to identify unmet needs, changes in condition, or caregiver strain to ensure care plans are updated accordingly.
• Collaborate with the interdisciplinary GUIDE care team to develop, implement and regularly update individualized care plans.
• Collaborate with the Participant and each GUIDE beneficiary and/or their caregiver to develop a person-centered care plan; ensuring care plans address both beneficiary and caregiver needs.
• Connect beneficiaries and caregivers to community-based services, including transportation, meals, adult day programs and social supports.
• Provide Caregiver Education and Support based on the caregiver assessment; this includes GUIDE Respite services, eligibility, and other caregiver needs.
• Coordinate and track referrals to approved respite providers.
• Accurately document all care navigation activities, caregiver contacts, assessments, referrals and interventions.
• Comply with all security and confidentiality regulations.
• Attend all required initial and ongoing trainings.
• Any other duties as assigned by Assistant Director of Community Health or COO.

Education:

Associates or Bachelor's (preferred) degree. 2+ years' experience in case management (preferred). Equivalent combination of education and experience.

Skills/Qualifications:

Ideal candidates will have at least two years of experience in care coordination or case management working with individuals living with dementia and their caregivers, and strong communication, assessment, and problem-solving skills.

• Strong customer service skills with the ability to be courteous and helpful.
• Team-oriented with willingness to collaborate.
• Strong listening and information gathering skills.
• Ability to work independently and assist with multiple projects, priorities, and locations.

• Proficient in the use of Microsoft Word and Excel.
• Good written and oral communication skills.
• Ability to meet deadlines.
• Ability to drive to a variety of locations.

To thrive as a Dementia Care Navigator, you need a background in health or social care, strong knowledge of dementia care, and experience working with individuals affected by dementia and their families. Familiarity with case management systems, referral procedures, and community resource databases. Relevant certifications in dementia care or social work can be advantageous. Outstanding interpersonal skills, compassion, and the ability to advocate and communicate effectively with clients and multidisciplinary teams help set you apart in this role. These skills ensure Dementia Care Navigators can guide and support families compassionately while efficiently connecting them to much-needed resources and services.

Special Requirements: Must pass DOEA Level II criminal background screening; must sign Medicaid Attestation Payroll Form per Department of Elder Affairs/AAAPP requirements. Must sign SMMCLTC Program – Prohibited Activities. For this position, driving is a regular component of the job, you will be required to have a non-expired Florida driver’s license for a Motor Vehicle Report (MVR) and must provide a copy of a valid driver’s insurance card. This position requires travel in Pasco and Pinellas Counties and a working vehicle is required.
