Job description

Minn Care Home Health is looking for Direct Support Professionals for our Assisted Living in New Hope, MN:

  • Monday through Thursday and every other weekend (7am - 3pm)

  • Starting at $20/hr

Minn Care Home Health provides home care services to adults with developmental and mental disabilities, and those in need of medical care after hospital discharge. The Direct Support Professional will perform direct client cares ordered by the client's physician and as documented in the client's care plan.

MAIN RESPONSIBILITIES:
A. Assist clients with personal care: dressing, grooming, bathing, eating, toileting, transferring, positioning, mobility.
B. Housekeeping: cleaning all living areas, making the bed, dishes, laundry, taking out the garbage
C. Monitor and re-direct client behaviors to ensure their safety.
D. Meal planning and preparation.
E. Medications: ordering, administration and monitoring.
F. Arrange client transportation.
G. Assist clients in getting around and participating in the community.
H. Assist clients in managing their finances.

ADMIN RESPONSIBILITIES:

  • Assist the House Manager with training and supervision of Direct Support Professionals.

  • Assist the House Manager with employee schedules.

  • Assist the House Manager with Resident schedules with healthcare providers (e.g., doctors, case managers) and replace staff as needed.

  • Assist the House Manager with developing and maintaining positive relationships with community health providers (e.g., community service organizations, hospitals, clinics, home health agencies) and other referral sources.

  • Assist the House Manager in planning and managing annual budget, building maintenance and emergency response system.

  • Assist the House Manager in developing, implementing and managing Assisted Living programs and services.

  • Supervise activities as directed by the House Manager and Program Manager.

  • Assist the House Manager with maintaining compliance with contractual and licensure obligations to vendors, residents, city, county, state and federal authorities.

  • Stay current on ongoing training as required by the state and federal government.

  • Maintain working knowledge of Assisted Living and Minn Care Home Health policies and procedures to ensure compliance.

REQUIREMENTS

A. Must pass the Department of Human Services background check
B. Must be reliable and have a strong work ethic
D. Fluent in English and have good communication skills
E. Must show compassion and empathy to clients
F. Must have a driver's license
