Job description

An experienced clinical pharmacist with strong expertise in EPIC is needed for a contract position located in Carbondale, CO. This role provides the opportunity to play an integral role in medication management during a critical EMR conversion from Meditech to Epic. The assignment begins as soon as possible and is projected to extend through August 15, 2026, with flexibility to adjust start and end dates.

Schedule/Hours:

  • Rotating day/evening shifts, 40 hours per week
  • Weekdays starting at 6:30 AM or 8:00 AM (Monday to Friday)
  • Rotating weekends starting at 7:00 AM to 12:00 PM

Key Responsibilities:

  • Accurately prepare and dispense medication per physician orders
  • Maintain medication stock in patient care areas
  • Ensure safe, cost-effective, and appropriate medication therapies adhering to policies and protocols
  • Interpret and transcribe medication orders into computerized patient profiles
  • Compound pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
  • Monitor drug therapy for contraindications, interactions, and appropriateness
  • Perform pharmacokinetic dosing and review culture & sensitivity data for antibiotic use
  • Detect and report adverse drug reactions promptly
  • Promote formulary compliance and therapeutic substitution protocols
  • Provide clinical consultation and drug information to medical staff, patients, and families
  • Document all clinical activities thoroughly
  • Participate in quality improvement and staff education programs
  • Serve as a member of the Code Response Team

Qualifications:

  • Active Colorado pharmacist license in good standing
  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
  • American Heart Association BLS and ACLS certification
  • EPIC system experience required; Meditech training is a plus
  • Flu vaccine required from October 1st through April 1st, no exemptions permitted
