Physical Therapist

Schedule: 8-hour shifts

Assignment Details:

  • Contract Length: 14 weeks
  • Guaranteed Hours: 32 per week

Requirements:

  • Current State License as a Physical Therapist
  • Proof of current CPR certification
  • Current driver's license with reliable transportation and insurance
  • 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
  • Previous home care experience (preferred)
  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

  • Perform clinical assessments to identify patient needs and appropriateness for physical therapy.
  • Document and review patient care plans, progress notes, and clinical records consistently.
  • Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on assessments and medical referrals.
  • Instruct patients and families on safe transfer techniques and the use of mobility equipment.
  • Provide patient and family education regarding therapeutic procedures based on individual needs.
