The pay for this position is $49,335-$64,798 annually, which is inclusive of a shift differential.

$500.00 sign on bonus

Second Shift Position (1–9pm)

The Care Navigator Case Manager leads service planning and care coordination navigation for children and families receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners.
