DEPARTMENT SUMMARY

The TGI Health Services department delivers coordinated, culturally responsive, and affirming care for transgender, gender diverse, and intersex communities through integrated clinical services, community programs, navigation, partnerships, evaluation, and referral systems. It operates within the Los Angeles LGBT Center and collaborates with external partners to enhance access and quality of care.

JOB SUMMARY

The Manager, TGI Health Systems will help shape a more coordinated, accessible, and affirming system of care for transgender, gender diverse, and intersex communities across the Los Angeles LGBT Center and the broader Los Angeles care landscape. This role serves as an operational integrator, connecting clinical services, community programs, navigation, partnerships, evaluation, and referral pathways so clients experience gender-affirming care as seamless, culturally responsive, and whole-person centered. Grounded in the Center’s community health model, the Manager will translate shared goals into practical workflows, timelines, tools, and accountability structures that strengthen access to care within the Center and across external systems of support.

ESSENTIAL FUNCTIONS

Workplan Execution & Project Management

•    Execute CDPH-approved workplan across all grant phases.

•    Maintain centralized project plan tracking activities, timelines, owners, dependencies, deliverables, and status.

•    Monitor progress of TGI clinical service expansion, staff onboarding, training, care coordination, navigation, evaluation, reporting, and partner activities.

•    Identify delays, risks, dependencies, or operational barriers and escalate with recommended solutions.

Cross-Functional Integration

• Coordinate across health services, community programs, evaluation staff, subcontractors, and external partners to support grant workplan implementation.

•    Facilitate alignment between clinical services and community programs for coordinated, whole-person care.

•    Clarify roles, responsibilities, handoffs, and dependencies among contributors.

•    Support operational consistency in service delivery across sites, teams, and programs.

Partnership Coordination

• Coordinate implementation of partnership with TransLatin@ Coalition by tracking deliverables, timelines, and communication.

•    Ensure outreach, navigation, workshops, and partner activities comply with grant requirements and timelines.

•    Facilitate communication between organizations and surface coordination issues requiring leadership attention.

•    Support subcontractors and contractors with grant compliance and reporting.

Grant Compliance, Reporting & Fiscal Support

•    Serve as day-to-day program contact for CDPH coordination, information requests, updates, and reporting support.

•    Maintain documentation and tracking systems for CDPH compliance, deliverables, program updates, and funder reporting.

•    Coordinate inputs for progress reports, implementation updates, and grant deliverables.

•    Support fiscal tracking by coordinating with finance and program leadership on grant-funded activities, spending alignment, budget-to- actual status, and reimbursement documentation.

Evaluation & Data Coordination

•    Support development and implementation of evaluation plan including data collection, reporting workflows, and partner coordination.

•    Partner with evaluation staff to track performance metrics, outputs, outcomes, and progress toward grant goals.

•    Prepare data, narrative content, and implementation updates for reporting.

•    Ensure evaluation activities integrate across clinical services, community programs, navigation, referral improvement, and partner activities.

Operational Coordination & Care Integration

•    Support day-to-day coordination of care pathways including referrals, navigation workflows, handoffs, and communication across teams.

•    Identify operational gaps, duplications, or inefficiencies and help implement improvements to strengthen coordination and client experience.

•    Support workflows connecting providers, navigators, community program staff, referral systems, and external partners.

Health Equity & Community Health Lens

•    Ensure implementation reflects understanding of barriers to care and social determinants of health affecting TGI patients.

•    Support coordination with programs addressing clinical and social needs to enhance access, continuity, and outcomes.

•    Promote client-centered, culturally responsive, and equity-centered implementation across workplan activities.

Internal Communication & Stakeholder Support

•    Facilitate project meetings, maintain clear communication across stakeholders, and ensure follow-up on action items.

•    Prepare materials for leadership updates, partner meetings, and funder discussions.

•    Support leadership by surfacing implementation risks, coordination issues, resource needs, and improvement opportunities.

NON-ESSENTIAL FUNCTIONS

Perform other duties as assigned

DECISION-MAKING SCOPE

Makes routine operational decisions related to project management and coordination within the grant workplan. Escalates risks, delays, and operational barriers with recommended solutions to leadership. Does not have independent budget, staffing, or policy decision authority.

SUPERVISORY RESPONSIBILITIES

No formal supervisory responsibilities. Coordinates workflows and supports team collaboration without direct staff management or performance evaluation authority.

REQUIRED QUALIFICATIONS

•    Experience managing complex, multi-stakeholder health services projects.

•    Familiarity with transgender, gender diverse, and intersex health care and community programs.

•    Knowledge of grant compliance, reporting, and fiscal monitoring processes.

•    Skills in project management including workplan development, tracking, and risk escalation.

•    Ability to coordinate cross-functional teams and external partnerships.

•    Experience supporting evaluation and data collection activities.

•    Proficiency with documentation and tracking systems related to program management.

•    Knowledge of grant compliance, reporting, fiscal monitoring, and transgender, gender diverse, and intersex health care contexts.

PREFERRED QUALIFICATIONS

•    Experience working with California Department of Public Health grants.

•    Familiarity with community health models and health equity principles.

•    Prior experience in nonprofit healthcare or LGBT-focused organizations.

•    Knowledge of specialty referral processes and patient satisfaction assessment tools.

•    Experience developing and delivering SOGIE trainings.

Physical Requirements

Prolonged sitting and computer use for project management and communication tasks. Frequent communication with internal and external stakeholders. Occasional travel or mobility may be required to support coordination activities.

Office environment with cross-functional collaboration. Fast-paced, multi- stakeholder project coordination. Confidentiality expectations related to sensitive health and community information. Possible hybrid or remote work arrangements depending on organizational policies.
