Whatย Youโ€™llย Doย 

As a PRN Hospice Registered Nurse (RN), you will:

  • Performs nursing assessments during scheduled and as-needed hospice visits to evaluate the patientโ€™s physical condition, symptoms, comfort, and immediate care needs.
  • Assesses the patientโ€™s and familyโ€™s response to the established hospice plan of care and communicates significant changes in condition or needs to the RN Case Manager and appropriate members of the interdisciplinary group (IDG).
  • Provides skilled nursing care in accordance with the established plan of care, physician orders, hospice policies, and nursing standards of practice.
  • Responds to changes in patient condition and provides appropriate nursing interventions to promote comfort, symptom control, and quality of life.
  • Administers medications and treatments as prescribed by the physician and evaluates the patientโ€™s response to treatment.
  • Collaborates with the RN Case Manager, IDG members, physicians, and other care providers to support continuity of care and ensure patient and family needs are addressed.
  • Provides PRN coverage based on patient needs and agency staffing requirements, including visits for symptom changes, medication concerns, changes in condition, and other skilled nursing needs as assigned.

  • Graduate of an accredited Registered Nurse (RN) programย 
  • Active South Carolina RN license or Compact Licenseย 
  • Minimum one year of nursing experienceย 
  • Hospice experience preferredย
