Job description

Position: Temporary - Registered Nurse (RN)
CareNational is seeking a skilled Registered Nurse eager for the opportunity to make a meaningful impact in patients' lives. This role plays a critical part in providing access to comprehensive care for underserved patient populations. By delivering in-home and virtual care, this RN position supports patients in managing chronic illnesses, including chronic kidney disease (CKD) and end-stage renal disease (ESRD), ultimately improving their quality of life and enabling positive health outcomes.
Your Impact
As a Registered Nurse (RN) in this role, you will provide personalized and compassionate care, primarily for patients with CKD through ESRD. You will serve as a floating resource in high-growth areas, ensuring patients receive the assessments, education, and care coordination needed to improve their health outcomes.
Your responsibilities will include:
  • Conducting patient health assessments to create strategies for disease management and health promotion.
  • Performing in-home and virtual visits, including annual wellness exams and focused screenings.
  • Educating patients and families on preventive care, treatment plans, and available health programs.
  • Documenting patient medical history, current medications, allergies, assessments, and treatment plans.
  • Coordinating care with providers, specialists, and integrated care teams to close gaps in care.
  • Enrolling patients in services and scheduling follow-ups to ensure continuity of care.

Roles & Responsibilities
  • Complete 3+ patient visits daily, both in-home and virtual.
  • Educate patients about preventive care and chronic disease management.
  • Maintain timely, accurate documentation in EMR systems.
  • Participate in integrated care team meetings to ensure care coordination.
  • Ensure compliance with quality-of-care standards and all regulatory requirements.
  • Assist patients in accessing educational resources to support their health.
  • Evaluate patient health status and adjust care plans as necessary.
  • Other duties as assigned.
Requirements
  • Active and unrestricted RN license in the state of practice.
  • 2+ years of direct patient care experience (managed care experience preferred).
  • Reliable transportation, valid driver's license, and car insurance.
  • Proficiency with EMRs, data management systems, and virtual care technology.
  • Ability to communicate effectively with patients, families, providers, and care teams.
  • Knowledge of federal and state regulations, including HIPAA and OSHA compliance.
