Sage Group HealthCare Staffing is seeking Home Health OT for a 13-week travel/contract positions in Everret, WA. We are accepting both local and travelers candidates.
Position Details:
Position Name: Occupational Therapist (OT) Home Health
Location: Everett, Washington
Specialty: Home Health

Contract Length: 13 weeks
Pay Rate: Range $2600 to $2800 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health
Shifts available: Days
Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:
  • Occupational Therapist: 1-2 years - Required
  • Recent/Current Home Health OT experience: 1 year within the last 3 years - Required
  • Home Care Home Base (HCHB)charting - Required
  • Must have valid Driver's Licenseand Personal Auto Insurance
Education, License and Certification Requirements:
  • OT License for State applying or Compact if applicable -Required
  • BLS Certification from AHA orAmerican Red Cross - Required
Job Description:
  • Provide OT services to Home Health patients
  • Delegate responsibility to the Occupational Therapist Assistant and Aides. Perform assessment, planning, intervention, and evaluation in accordance with occupational therapy standards, physician orders, and governing policies and procedures.
  • Evaluate the patient's function by performing formal/informal testing measures to determine the skeletal, neurological, respiratory, cardiovascular status, developmental, social, and cognitive functions
  • Develop a goal specific treatment regime based on assessment data and the patient's tolerance to carry-out the treatment plan as prescribed.
  • Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status.
  • Devises a schedule of treatment, which is frequency and duration specific to be discussed with patient/family/caregiver and coordinated with clinical support staff.
  • Educates patient, family, and/or caregiver regarding pathology or condition, patient alternatives, prognosis, and rationale for treatment through forms of rest, exercise, modalities, and other indicated interventions.
