Responsibilities

  • Provide patient care and basic nursing tasks within LTC setting per protocols.

  • Monitor patient status; document vitals, changes, and care plans accurately.

  • Administer medications and treatments as prescribed; track adherence and side effects.

  • Ensure compliance with safety, infection control, and regulatory standards.

  • Collaborate with interdisciplinary team to update care plans and discharge planning.

  • Identify and report clinical changes early; escalate to supervising RN as needed.

  • Demonstrate accountability for personal performance; meet individual targets and quality metrics.

  • Support professional development through ongoing learning and skill enhancement.

  • Maintain organized patient records, optimize workflow, and improve care continuity.

  • Engage with residents and families to provide compassionate communication and education.

  • Participate in daily huddles and pipeline reviews to forecast care needs and staffing.
