Job description

Home Health Nurse

We are seeking dedicated and clinically sound RNs and LPNs to provide skilled nursing care to patients in their homes. This role includes intermittent visits, private duty shifts, and care plan coordination. If you're licensed in Florida, reliable, and looking to make a meaningful impact in a flexible per diem role, we'd love to connect.

Key Responsibilities

  • Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments
  • Carry out shift-based care for patients needing extended clinical support
  • Monitor and document patient condition and progress
  • Collaborate with physicians and interdisciplinary teams to update care plans
  • Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation required for RN)
  • Uphold infection control standards and agency protocols

Qualifications

  • Active Florida RN or LPN license (Compact License accepted)
  • Current CPR certification (BLS)
  • Level 2 background screening (AHCA-compliant)
  • TB screening and physical (within past 6 months)
  • Experience in Medicare-certified home health strongly preferred
  • Familiarity with EMRs (e.g., Axxess) and knowledge of PDGM & OASIS for RNs
  • Excellent clinical judgment, communication skills, and time management
  • Reliable transportation and commitment to punctual, professional care
