Job Brief
Responsible for nursing assessments, writing of diagnoses, developing and intervening in the individuals receiving services health care plans to ensure quality care in the most efficient manner.

POSITION SUMMARY:
Responsible for nursing assessments, writing of diagnoses, developing and intervening in the individuals receiving services health care plans to ensure quality care in the most efficient manner.
POSITION DUTIES:
  1. Develops and updates health care plans to address assessed needs.
  2. Provides direct medical/nursing services to assigned individuals as presented.
  3. Assists with administration of specialized medication such as insulin and g-tubes.
  4. Provides staff training, pours, and other things including but not limited to; attending IDT trainings, counseling for medical errors, discharge meetings, and communications with other facilities.
  5. Ensures proper documentation in Precision Care of persons receiving service care and progress.
  6. Details acute and chronic health problems and interventions; reviews, medication, hospitalizations, clinics, dietary concerns, etc.
  7. Identifies the need for and arranges conferences to facilitate resolution of complex issues.
  8. Participates in chart reviews and follow up for internal surveys.
  9. Conducts on-going reviews of medication administration documentation.
  10. Flexible to other duties as needed.

REQUIREMENTS:
  1. NYS Registered Nurse License and Registration
  2. Active Basic Life Support (BLS) Certification
  3. Bachelors of Associates in Nursing
  4. CPR Certification Preferred
  5. 1-2 years of experience working with individuals with developmental disabilities
  6. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency's corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
