Job Title: Registered Nurse – DDD Habilitation Nurse
Location: Tucson, AZ – District South Area
Type: Contract
Shift & Schedule: Days | Monday – Friday | 8:00 AM – 5:00 PM | 45 Hours Per Week
Work Setting: Field-Based / Travel Required
Pay: $30/hr - $35/hr

Position Overview:

We are seeking experienced Registered Nurses to provide clinical support, conduct nursing assessments, coordinate services, and support DDD members across the assigned service area.

Responsibilities

  • Conduct comprehensive nursing assessments for DDD members.

  • Complete in-person assessments every 90 days and as needed.

  • Evaluate and document the medical necessity of skilled nursing services.

  • Prepare accurate nursing reports and documentation.

  • Coordinate with members, families, caregivers, providers, and external agencies.

  • Assist with referrals and coordinate medical and nursing services.

  • Monitor changing health conditions and identify potential risks or unmet needs.

  • Advocate for members' rights, health, safety, and individualized service needs.

  • Participate in multidisciplinary and interprofessional planning meetings.

  • Provide temporary nursing coverage and support across the assigned district as needed.

  • Maintain accurate records and required documentation.

  • Participate in mandatory meetings, trainings, and annual educational requirements.

  • Follow DDD, AHCCCS, and applicable state policies, procedures, and clinical guidelines.

  • Travel throughout the assigned service area as required.
