Triage Desk Registered Nurse (RN) - Family Medicine
Houston, TX (77014) | $40/hour | Contract (Temporary)
Monday-Friday, 8:00 AM - 5:00 PM
A leading physician group in Houston is seeking a skilled and patient-focused Triage Desk Registered Nurse (RN) to support its Family Medicine team on a temporary contract basis. This role is primarily administrative and focused on clinical coordination, serving as a key point of contact between patients and providers. The Desk RN will triage patient concerns, review charts, manage communications, and ensure continuity of care through efficient coordination and documentation.
Position Overview
This role is ideal for an RN who thrives in a fast-paced outpatient setting and enjoys patient interaction, critical thinking, and care coordination. You will play a key role in supporting providers and ensuring patients receive timely guidance and follow-up care.
Key Responsibilities
  • Review daily provider schedules and patient charts
  • Answer and triage patient phone calls and portal messages
  • Assess adult and family medicine symptoms and determine next steps using established protocols
  • Communicate patient concerns to providers and relay care instructions to patients
  • Review lab and imaging results and flag abnormalities for provider review
  • Document all calls, messages, and coordination activities in Epic EMR
  • Coordinate referrals, prior authorizations, and follow-up appointments
  • Provide patient education (medications, chronic conditions, preventive care, red flags) via phone or portal
  • Collaborate with providers and clinic staff to support efficient patient flow
  • Complete administrative and care coordination tasks related to patient management

Required Qualifications
  • Active Registered Nurse (RN) license in the state of Texas
  • Bachelor of Science in Nursing (BSN)
  • Previous RN experience (clinic, triage, or ambulatory setting preferred)
  • Proficiency with Epic EMR
  • Strong communication, critical thinking, and organizational skills
  • Active BLS certification
