Occasional earlier start times as needed
No evenings or overnight shifts
This role is open exclusively to New York State–licensed Registered Nurses (RN).
Job Overview:
We are seeking a compassionate and organized Registered Nurse (RN) Case Manager to provide healthcare oversight and coordination for individuals with intellectual and developmental disabilities in residential settings throughout the Hudson Valley.
This role focuses on nursing assessments, care coordination, and collaboration with interdisciplinary teams, offering an excellent opportunity for RNs interested in community-based care rather than a hospital setting.
RN Responsibilities:
  • Coordinate and oversee healthcare services for individuals in group homes
  • Perform nursing assessments and clinical evaluations
  • Monitor overall health status and ensure continuity of care
