Home Health Registered Nurse (RN) – Field

Full-Time | Flexible Schedule | Autonomy + Support

Tired of rushed hospital shifts and high patient ratios? As a Home Health RN, you’ll deliver 1:1 care where it matters most—your patient’s home. You’ll have the autonomy to make clinical decisions, build meaningful relationships, and truly see the impact of your care.


What You’ll Do

As a Field RN, you’ll manage your own caseload and provide skilled nursing care across the full patient journey:

  • Perform start-of-care, recertification, and routine skilled nursing visits
  • Conduct comprehensive assessments (physical, psychosocial, environmental)
  • Develop and manage individualized Plans of Care
  • Educate patients and families on disease management, medications, and self-care
  • Identify changes in condition and coordinate care with physicians and the care team
  • Supervise and provide direction to LVNs/LPNs and Home Health Aides
  • Ensure accurate, timely clinical documentation (HomeCare HomeBase preferred)
  • Maintain compliance with Medicare/insurance guidelines
  • Participate in on-call rotation

What You Bring
  • Active RN license
  • Valid driver’s license & auto insurance
  • CPR certification
  • 2+ years of Home Health experience preferred (strong clinical RNs encouraged to apply)
  • Experience with HomeCare HomeBase is a plus
