Overview
Contributes to the development, maintenance, or modifications to the individual service plans. Identifies changes in condition with proper documentation according to nursing practice standards. Demonstrates consistent follow through for positive resident outcomes. Utilizes good judgment and specialized skills.
What you will do
  • Works under the supervision of a Registered Nurse, Nurse Manager or Nursing Supervisor, and ultimately responsible to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The Licensed Practical Nurse (LPN) monitors the resident. Participates in the planning, implementation and evaluation of nursing care. Provides for the emotional, physical comfort and safety of the residents and their families. Assists in supervision of auxiliary personnel. Performs other duties as assigned.
  • Allows resident choices within limits of capability considering safety /dignity issues and follows policies and procedures of facility to include resident privacy and dignity.
  • Assigns duties to non-professional staff such as CNAs according to education, training, policy and resident need in order to assure quality resident care is delivered. Engages team members for optimal performance and addresses performance as needed.
  • Contributes to the development, maintenance, or modifications to the individual service plans. Identifies changes in condition with proper documentation according to nursing practice standards. Demonstrates consistent follow through for positive resident outcomes. Utilizes good judgment and specialized skills.

Qualifications
Experience
  • Less than 1 year Nursing experience (Preferred)

Licenses and Certifications
  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required) and
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
