Job description
ESSENTIALÂ
Provides personal aspects of care to patients in order to increase level of comfort and maintain personal hygiene. Â
Provides temporary respite and relief to patient’s family from total personal care of the patient.Â
Assists in maintaining a safe, healthy environment for patients and families. Â
Meets with Interdisciplinary Team, when appropriate, to assists with review and amendment of the patient’s care plan. Â
Practices safely and competently within job description. Â
Completes duties according to the Hospice Aide assignment sheet. Â
Reports significant changes in patient’s condition to registered nurse (done so for follow- through and appropriate intervention). Â
Maintains accurate and up to date records of visits and duties performed per Hospice documentation policies. Â
Provides personal care (ie: bathing, oral hygiene, skin care, hair care, bath rules, changing of bed linen, checking of vital signs under supervision of primary nurse).Â
Transfer of patient (ie: bed to chair, bed to commode, chair/commode to bed).Â
Turning and positioning of patients.Â
Assisting with bed pan/urinal.Â
Assist with ambulation and exercise of patient as ordered and so assigned.Â
Maintain clean, safe, and comfortable environment.Â
Simple wound care, removal of impaction, and providing Fleet’s enema under RN supervision.Â
Education:
- High School Diploma or GED preferred.
Education:
- Completion of approved nurse aide training program and successful completion of competency evaluation.
- Current CPR certification
Experience:
At minimum, one year experience as a Nurse's aide.
Skills:
- Sensitivity and openness to the physical needs/care of the terminally ill.
- Maturity and ability to deal effectively with demands of the job.
- Must have the ability to read, write, and carry out directions promptly and accurately.
- Good physical and emotional health.
Other Requirements:Â
- Must have own transportation and provide proof of liability insurance which meets state requirements.
- Must be able to travel to and from all patient homes without assistance.
- Must be able to perform all aspects of the position unassisted.
- Must have good communication skills and techniques with facilities, patients, families, physicians, and office staff.
