Join a mission-driven team dedicated to improving the health outcomes for youth/young adults and families experiencing homelessness in Greater Boston. The Adult Health RN provides compassionate, comprehensive, and developmentally appropriate care to youth/young adults, and families experiencing homelessness across Greater Boston. This role blends hands-on nursing with shared operational coordination, ensuring smooth clinical workflows, regulatory compliance, and quality improvement across sites.
While primarily focused on the adult and youth/young adult populations, the Adult Health RN also provides pediatric care as needed and maintains competency in family-centered nursing to ensure continuity across all ages. This position also provides coordination for youth and young adult populations, ensuring smooth transitions from pediatric to adult care and integrating outreach, education, and preventive health services tailored to this age group.
Key responsibilities include:
  • Delivering pediatric and adult patient care, including assessments, interventions, and health education
  • Managing quality improvement initiatives, regulatory compliance, and vaccine program
  • Serving as vaccine manager for the adult population
  • Supporting public health measures tailored to the needs of youth and families experiencing homelessness

The Family Team provides integrated health services, including primary and episodic care, immunizations, mental health and substance use treatment, family planning, nutrition, and social service advocacy to pediatric, young adults, and adult patients. Care is delivered at the Family Clinic and various shelters, daycare centers, housing, and outreach locations throughout Greater Boston.

Responsibilities:
Direct Patient Care
  • Provide direct pediatric and adult care, including assessments, immunizations, developing and implementing interventions, health education, monitoring progress, and evaluating effectiveness
  • Conduct triage and walk-in visits at Family Clinic and outreach locations
  • Address health gaps and concerns for patients and their families
  • Collaborate with case managers to address health needs
  • Provide health education focused on preventative care, disease management, and reproductive health
  • Collaborate with Pediatric Health RN to ensure seamless care for all patients served by the Family Team
Clinical and Operational Responsibilities
  • Serve as Vaccine Manager for adult vaccine programs, ensuring inventory management, compliance, and reporting
  • Lead coordination for youth and young adult populations, ensuring smooth transitions to adult care and integrating outreach, education, and preventive health services tailed to this age group
  • Lead and support quality improvement initiatives; assist with data monitoring and reporting
  • Collaborate with Pediatric Health RN and Family Team leadership to develop and refine clinical protocols
  • Support clinical operations and supply management to ensure adherence to local, state, and federal healthcare regulations
  • Assist in the transportation and distribution of supplies, medications, and vaccines between various sites
  • Serve as co-liaison for regulatory and public health programs (e.g., infection prevention, adult immunization initiatives, chronic disease management)
  • Participates in public health efforts tailored to the needs of families and youth experiencing homelessness
  • Contribute to policy and procedure development and staff education in collaboration with Family Team and BHCHP staff
Qualifications:
  • Registered Nurse with a current Massachusetts license
  • BSN required; community or pediatric health experience preferred
  • Minimum of three (3) years of nursing experience
  • Valid driver's license and access to a car for travel between sites required
  • Leadership experience with demonstrated management ability
  • Experience working with underserved populations, including youth and families experiencing homelessness
  • Outreach nursing experience preferred
  • Bilingual proficiency in Spanish, Haitian Creole, or other languages commonly spoken by the population served. Additional salary compensation is available for certified proficiency

Compensation and Benefits:
***NOW OFFERING A $4,000 SIGN ON BONUS***
  • The compensation starts at $40.00 - $58.00 per hour and actual rate is based on years of experience.
  • BHCHP full time employees are eligible for our competitive time off program, health, dental and vision insurance, 403B retirement savings plan, pre-tax MBTA pass program with 40% discount, additional compensation for demonstrated bilingual proficiency and more. Benefits are prorated for part-time employees.
