We are seeking a compassionate and experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes in San Diego, CA.
Key Responsibilities

  • Perform skilled nursing visits and comprehensive in-home patient assessments.
  • Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments.
  • Develop, implement, and update individualized care plans.
  • Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.
  • Educate patients and caregivers on disease management, medications, and home safety.
  • Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
  • Maintain accurate and timely documentation in the EMR.

Minimum Requirements

  • Minimum 1 year of recent Home Health RN experience
  • Experience caring for geriatric patients
  • Current BLS/CPR Certification
  • OASIS experience required
  • Active California RN License preferred (pending CA license accepted if application has been submitted prior to submission)
  • Valid Driver's License, reliable transportation, and current auto insurance
