Nurse Practitioner (NP) / Physician Assistant (PA)

Location: Decatur / Springfield, IL

Job Type: Full-Time

Compensation: $180,000 annually (base salary + incentives).

Schedule:

  • Monday – Friday
  • 40 hours per week
  • No weekends
  • No holidays

Overview:

Join a mobile healthcare team providing primary care, wellness visits, and chronic disease management for geriatric patients in assisted living and memory care communities. This autonomous role offers the opportunity to build meaningful patient relationships while delivering high-quality, patient-centered care in community-based settings.

Responsibilities:

  • Provide comprehensive primary care services to geriatric patients.
  • Conduct wellness visits, chronic disease management, and preventive care.
  • Assess, diagnose, and develop treatment plans for patients.
