Key Responsibilities

• Conduct thorough evaluations of patients, reviewing medical histories, performing assessments, and documenting findings comprehensively.
• Develop personalized treatment plans in collaboration with patients and their families, ensuring that goals align with the patients' needs and expectations.
• Utilize evidence-based practices and demonstrate proficiency in implementing treatment protocols that adjust according to each patient’s progress and evolving healthcare standards.
• Communicate effectively with a multidisciplinary team of healthcare professionals to modify care plans as necessary, ensuring comprehensive patient support and timely intervention.
• Facilitate discharge planning and follow-up care, ensuring that necessary documentation reflects the patient’s journey, including progress notes and discharge summaries.
• Demonstrate exemplary time management skills to meet productivity metrics while maintaining high standards of care and accuracy in scheduling.
• Engage in community outreach and marketing efforts to promote our services, establishing connections within the local healthcare network.
• Participate actively in departmental goal-setting and compliance with regulatory standards, contributing to the overall success of the organization.
• Mentor and supervise students and newly hired staff, guiding them in best practices and encouraging professional growth.
• Enhance patient access to care by participating in initiatives aimed at removing barriers and improving service delivery.

Patient Population / Client Focus

At Swift Health Physical Therapy, we serve a diverse patient demographic, including individuals of all ages with varying physical rehabilitation needs. Our clients may present with orthopedic injuries, neurological disorders, post-operative rehabilitation needs, and chronic pain conditions. Qualifications

• Bachelor's Degree in Physical Therapy or a related field required, with a Doctorate in Physical Therapy (DPT) highly preferred.
• Current and valid Physical Therapist license in the state of Florida.

• Certification in Basic Life Support (BLS) for Healthcare Providers required.

Preferred Experience

• A minimum of 2 years of experience as a licensed Physical Therapist in a clinical setting is preferred, providing insights into diverse patient challenges.
• Previous experience in outpatient therapy environments and familiarity with electronic health record systems will be advantageous.

Compensation and Benefits

Swift Health Physical Therapy offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications. In addition to salary, we provide an extensive benefits package that includes:
