Position SummaryThe Supportive Housing Program Manager is responsible for oversight of the Supportive Housing programs, which include the HUD – COC Grants: Dual Diagnosis and PSH programs. The Program Manager has a reduced caseload of clients to process all rental assistance invoices and utility payments, as well as managing data quality in the HMIS data management system. This position supervises case managers of those programs and reports to the Vice President of Programs.
Duties and ResponsibilitiesManagement- Responsible for the oversight of HUD-funded housing programs and specialty housing for individuals with substance abuse and severe mental illness, and for individuals experiencing homelessness with possible criminal backgrounds.
- Effectively lead staff to include supporting core values, addressing zero tolerance behaviors, and maintaining active communication with direct reports.
- Create and maintain systems to ensure client records and databases are up to date, ensure that client interactions are timely and appropriately documented in the HMIS system, and adhere to appropriate confidentiality standards relative to data. Ensure Best Notes is utilized regularly to support the billing of supportive services.
- Administer all housing vouchers, manage client intake process, and conduct/design interviews, assessments, and applications of prospective clients for the program by following up on referrals from external and internal sources.
- Oversee and supervise and train new hires on the Supportive Housing staff, and responsible for the supervision and evaluation of all staff in the assigned HPP housing programs. Provide guidance and support with their tasks as needed.
- Ensure client program participation and adherence to HUD standards by facilitating all client tenant conferences and/or behavior contracts with assigned case manager and client.
- Work closely with the finance department to ensure smooth and timely payments of all monthly rental and utility payments and deposits.
- Create and maintain updated policies and procedures, training manuals for assigned housing programs, and oversee the creation and implementation of yearly training for all staff.
- Collaborate with community partners and other housing supportive service providers, and foster relationships with partnering agencies.
- Coordinate with Pikes Peak Continuum of Care Coordinated Entry and other entities as needed in order to facilitate contracts, referrals and program administration. Represent Homeward Pikes Peak during all Coordinated Entry meetings.
- Closely track all program changes reflecting client/program successes or shortfalls, and provide report monthly to CEO, Vice President of Programs, and Director of Development.
- Fulfill all responsibilities of case management staff when applicable to cover illness and vacations or vacancies.
- Manage donation inquiries for the program and maintain donor relationships. Report and track donations and submit to Director of Development as needed.
- Work with the Leadership Team to set or review strategies and short/long term goals for effective and efficient operations of the assigned programs.
- Oversee and track data in HMIS for outcome performance reports and run APR reports as needed.
- Manage and submit payments to community partners and vendors (i.e. Laundry Time).
- Other duties as assigned.
- Serve as Case Manager for client/residents (reduced caseload) to ensure program participation: conduct home visits with each client at a minimum of once a month; maintain weekly contact by phone; work with each client to set goals for healthy individual growth with an emphasis on maintaining recovery; monitor progress in attaining goals; monitor adherence to all Homeward Pikes Peak – Supportive Housing policies and procedures.
- Provide transportation for clients to appointments when necessary.
- Coordinate with other community service agencies to access services for clients as needed.
- Maintain accurate records, which include client charts as well as internal and external reports.
- Develop Individualized Services Plans for clients.
- Coordinate communication regarding housing needs between client, property manager, and Housing Specialist.
- Provide housing navigation, moving assistance and acquisition of furniture or other housing essentials on behalf of clients.
- Other duties assigned.
- Attend, lead, and participate in staff meetings.
- Attend and/or participate and/or interact with donors or fundraising as requested.
- Produce documentation timely as requested for HUD monitoring, financial audits, grant reports or other business requirements.
- Maintain work and office environment to professional standards.
- Proficiency in general computer skills and Microsoft Office applications including Word, Excel, and Outlook; and word processing, spreadsheets, CRM databases, and other case management data platforms.
- Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
- Demonstrated knowledge and training in generalized health care standards, substance abuse recovery process and treatment, mental health care, and trauma informed care skills.
- Professional written and verbal communication skills.

- Self-starter and ability to work independently with minimal supervision.
- Ability to occasionally lift or move up to 50 lbs.
- Bachelor’s level degree in Social Services required.
- Four (4) years of experience in the human service field providing direct client supportive services.
- Two (2) years of supervisory or management experience preferred.
- Experience working directly with people from diverse socioeconomic and ethnic backgrounds, ability to communicate effectively, crisis intervention, and harm reduction.
- Understanding of regulations and standards regarding privacy and confidentiality.
- Valid Colorado driver’s license, clean driving record, and must carry personal vehicle insurance. (must be able to drive with clients)
- Master’s level degree in Social Services preferred.
- Knowledge of Mental Health Diagnosis and addictions counselling, experience or certifications (such as LAC) preferred.
- Experience working for a non-profit organization.
- Spanish speaker preferred but not required.

Work EnvironmentThis position will be required to attend home visits of clients in our program. While performing the duties of this job, the employee may be exposed to airborne viruses, bacteria, or other bodily pathogens carried by residents. The employee is occasionally exposed to outside weather conditions and fumes or airborne particles when driving from one HPP location to another or participating in an outdoor resident social activity. The noise level in the work environment can range from quiet while in a private office to loud while driving, attending a large residential social event, or visiting resident common areas where there are televisions and equipment operating, phones ringing, light to heavy traffic, and people talking.
