RN – Case Management

Seven Healthcare are seeking an experienced RN – Case Management professional for a travel assignment in Fort Wayne, IN. This role focuses on inpatient case management within an acute care setting, emphasizing utilization review and comprehensive discharge planning. The selected candidate will work collaboratively within a hospital environment, utilizing systems such as Epic for documentation.

Key Responsibilities of the RN – Case Management

  • Conduct utilization review in collaboration with the care team
  • Complete comprehensive patient assessments to drive discharge
  • Drive stay progression and complete discharge planning
  • Provide thorough and accurate Epic documentation
  • Perform IP Case Management work across units as needed

Requirements of the RN – Case Management

  • IN or compact RN license required
  • Minimum of 2 years of inpatient case manager experience
  • Proficient in MCG and InterQual
  • Prefer certification in Case Management
  • BLS Certification
