Position Summary The Clinical Care Manager at Rose Garden plays a crucial role in prioritizing resident care and coordinating comprehensive care plans. Collaborating closely with the team, the Clinical Care Manager ensures the implementation of resident care plans and promptly addresses unexpected situations. The Clinical Care Manager works closely with the Clinical Care Staff Coordinator to assist in hiring and training staff, completion of competency training for new and existing team members and stepping in to address staffing needs when needed. Overall, the Clinical Care Manager is dedicated to enhancing resident care through continuous improvement initiatives.  


What this role would like day to day:

  • Assures appropriate documentation is completed for resident care plans, including the implementation of such plans.
  • Schedules and conducts care conferences.
  • Reviews and completes clinical documentation and paperwork prior to a potential admission.  
  • In response to staff absence, is expected to make up for staffing where there is a need, including working on the floor if necessary.
  • Standardizes charting for Supervisors, Med Tech, and RAs.  
  • Performs Infection Control rounds and is responsible for facility infection control. 
  • Meets as an Interdisciplinary team (IDT) as needed; Completes incident, med error, and falls forms in correlation with the IDT.  
  • Enhances current process and procedures by attending in-house educational services.
  • Collaborates with the Clinical Care Staff Coordinator to hire and train staff, including ongoing competency training.
  • Inputs Corporate and Facility QAPI information for quarterly meetings.  
  • Works as Supervisor or Med Tech as scheduled. 
  • Other functions as assigned.

We invite you to apply to work at our Rose Garden campus today! This position is working full-time, 32 hours per week. There is an opportunity for flexible scheduling! This includes hours and weekdays. As long as the days are within Monday - Friday and an 8-hour shift between the hours of 6 am - 5 pm. NO weekends or Holidays are required.  

Requirements

Qualifications and Experience Required:

License from the state of Michigan as a Licensed Practical Nurse preferred. 

Experience in department management.
