About the Team

The Pharmacy Department at Lakewood Health System provides comprehensive medication management services that support safe, effective, and cost-conscious drug therapy across inpatient and outpatient settings. The team works collaboratively with physicians, nursing staff, and interdisciplinary partners to enhance patient outcomes, ensure regulatory compliance, and promote continuous quality improvement.

To ensure safe, effective, and evidence-based medication therapy through collaborative clinical pharmacy services that support high-quality patient care and advance Lakewood Health System’s mission.

Position Summary

The Clinical Pharmacist plays a vital role in patient care by ensuring the safe, effective, and cost-conscious use of medications throughout the health system. This position combines direct clinical pharmacy services with operational oversight, working closely with physicians, nursing staff, and other healthcare professionals to optimize medication therapy. The Clinical Pharmacist monitors patient medication regimens, provides pharmacokinetic and drug information consults, supports antibiotic stewardship and nutrition services, and contributes to quality improvement initiatives. In addition, this role helps maintain regulatory compliance, supports pharmacy operations, and provides leadership and supervision to pharmacy support staff as needed.

Position Type

The is a full-time position working 64 hours a pay period (0.8FTE). Schedule and on-call expectations may vary based on department needs. Rotating weekends and holidays required. 

Qualifications

  • Graduate of an accredited College of Pharmacy
  • Current licensure as a Registered Pharmacist in the State of Minnesota
  • Doctor of Pharmacy (PharmD) degree or a minimum of three (3) years of hospital pharmacy experience
