Home Health Physical Therapist Signature Healthcare at Home is seeking a full‑time Physical Therapist to serve the Southeast Portland Metropolitan Area, including neighborhoods such as Sellwood, Woodstock, Lents, Montavilla, Happy Valley, and surrounding communities. This role is ideal for PTs who value autonomy, meaningful patient relationships, and the opportunity to deliver high‑quality care directly in patients’ homes.

Key Responsibilities

PT Responsibilities

  • Perform comprehensive physical therapy evaluations in patient homes.

  • Develop individualized treatment plans focused on functional mobility, safety, and independence.

  • Provide therapeutic interventions including gait training, strengthening, balance work, and home‑exercise program development.

  • Conduct home‑safety assessments and recommend appropriate DME or environmental modifications.

  • Manage a field‑based caseload with strong organizational efficiency.

  • Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.

  • Maintain accurate, timely documentation in accordance with regulatory standards.

  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Qualifications

PT Requirements

  • Current Oregon Physical Therapist license (or ability to obtain).

  • One year of PT experience preferred; home‑health experience a plus.

  • Strong communication, assessment, and clinical reasoning skills.

  • Ability to work independently in a field‑based environment.

  • Reliable transportation and valid driver’s license.
