This position is remote, allowing you to effectively engage with patients and the rehabilitation team from the comfort of your home. However, you will be connected deeply to our hospice center’s workflows, utilizing platforms like telehealth systems for assessments and counseling sessions. • Conduct thorough psychosocial assessments, employing interviews, clinical observations, and patient medical records to identify emotional, social, and environmental barriers affecting recovery.
• Provide essential counseling and support to patients and their families, skillfully addressing adjustment issues related to health transitions and emotional well-being.
• Collaborate with a team of healthcare professionals, including physical and occupational therapists, to create individualized treatment plans that accommodate both psychological and functional needs.
• Assist patients in accessing crucial community resources, including support groups, vocational rehabilitation services, and housing assistance to enhance their transition process.
• Educate patients and families concerning available community resources and post-rehabilitation services, helping them navigate the next steps in their recovery.
• Participate actively in discharge planning, facilitating seamless transitions from our hospice center to home or other care settings while ensuring that patient needs continue to be met effectively.

Your work will primarily focus on patients recovering from significant health incidents, including strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. The diverse backgrounds and varying needs of our clientele require a tailored approach to care, making it crucial that you understand the dynamics of grief, loss, and coping strategies associated with rehabilitation.

• A Master's degree in Social Work from an accredited program.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong theoretical and practical knowledge of psychosocial factors and their impact on health recovery.
• Exceptional communication and interpersonal skills to foster relationships with patients, families, and colleagues.

• Prior experience in a rehabilitation or medical setting is strongly preferred, as it provides valuable insights into the unique challenges faced by patients in these environments.
• A solid understanding of psychological adjustment issues, grief, and effective coping mechanisms is essential for quality patient care.
