Key Responsibilities
  • Conduct thorough psychosocial assessments, employing interviews, clinical observations, and patient medical records to identify emotional, social, and environmental barriers affecting recovery.
  • Provide essential counseling and support to patients and their families, skillfully addressing adjustment issues related to health transitions and emotional well-being.
  • Collaborate with a team of healthcare professionals, including physical and occupational therapists, to create individualized treatment plans that accommodate both psychological and functional needs.
  • Assist patients in accessing crucial community resources, including support groups, vocational rehabilitation services, and housing assistance to enhance their transition process.
