We are hiring Nurses and Social Workers for corporate and field opportunities in several areas:
• Transitional Care
• Behavioral Health

These positions will provide non-clinical and case management services to promote the self-management of chronic diseases to members with special health care needs. Applicants with behavioral/mental health/psychiatric, pediatric, and case/care management experience are especially encouraged to apply.
