NOW HIRING: Social Worker

Responsibilities:
Conduct comprehensive psychosocial assessments of residents to identify their emotional, social, and environmental needs.
  • Develop individualized care plans in collaboration with interdisciplinary teams to address residents' social and emotional concerns.
  • Provide counseling and emotional support to residents and their families, especially during times of transition or crisis.
  • Coordinate discharge planning and arrange for appropriate community resources and services for residents transitioning to other care settings.
  • Advocate for residents' rights and ensure their voices are heard in care planning meetings.
  • Collaborate with healthcare professionals, including nurses, physicians, and therapists, to ensure holistic care for residents.
  • Document assessments, interventions, and progress notes accurately in resident records.

Qualifications:
