JOB SUMMARY:

PeopleOne Health has transformed healthcare by bringing together primary care, medical specialists, motivational coaching, and behavior change technology. The Licensed Clinical Social Worker position is an integral part of the behavioral health care team and will work directly with the clinical care team and health coaches to meet our members' mental and behavioral health needs. This position will serve the PeopleOne Health offices in their designated area.

SUPERVISORY RESPONSIBILITIES:

ESSENTIAL JOB FUNCTIONS:

  • Support clinical care team with SBIRT implementation (Social Worker will be fully trained in SBIRT)
  • Engage in care coordination for members to access our therapy and psychiatry partnerships to connect members with mental health services
  • Collaborate with a multidisciplinary care team in service of members
  • Provide Social Determinants of Health screenings, resources, and referrals
  • Engage in community outreach to build relationships and partner with local organizations
  • Coordinate with local substance use treatment programming for member assessments and referrals
  • Provide education for members and employees on current standards of care in mental health

Requirements

SKILLS & ABILITIES:

  • Excellent verbal and written communication skills
  • Excellent interpersonal and customer service skills
  • Demonstrates authenticity and empathy in engagement with members
  • Able to work well under pressure and stay calm/focused in emergency situations
  • Knowledge of EHR is a plus
  • Practices cultural humility
  • Ensures privacy and confidentiality as HIPAA requires and adheres to company and departmental guidelines, policies, and procedures.
  • Comfortable working independently as well as part of a diverse group of people.ย

EDUCATION & CERTIFICATIONS :

  • Master's degree in social work (MSW) required from an accredited institutionย 
  • State Licensure as a Clinical Social Worker
  • Specialized certification as Licensed Clinical Social Worker (LCSW)

EXPERIENCE:

  • 1-3 years post-Bachelor's experience preferred
  • Knowledge of community resources required
  • Experience working in the field of mental and behavioral health required
