• Conduct thorough psychosocial assessments to evaluate the strengths and needs of hospice clients and their families, ensuring a holistic understanding of their situations.
• Offer evidence-based therapeutic services, including individual and group therapy tailored to meet the unique needs of individuals facing end-of-life challenges.
• Collaborate closely with interdisciplinary healthcare teams, including nurses, physicians, and hospice aides, to create comprehensive care plans that address the emotional, social, and spiritual needs of clients.
• Advocate for clients by connecting them with community resources and support services such as financial aid, support groups, and additional mental health resources.
• Maintain accurate and up-to-date documentation of client interactions, treatment progress, and outcomes in electronic health records, adhering to regulatory and ethical standards.
• Participate in team meetings, case conferences, and ongoing professional education to enhance your clinical skills and contribute to collective team knowledge.

Your role will primarily focus on hospice patients and their families, who may be facing significant emotional distress due to terminal diagnoses and impending loss. You will provide compassionate care that not only addresses immediate mental health needs but also supports clients in navigating grief, acceptance, and adjustment to life changes. Understanding cultural sensitivities and tailoring approaches to diverse populations is key to fostering trust and effective communication.

• A Master’s degree in Social Work from an accredited program is required for this position.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon is a must.
• A minimum of two (2) years of post-licensure experience directly providing clinical social work services is essential.
• Proficiency in conducting diagnostic assessments and formulating comprehensive treatment plans is necessary.
• Demonstrated strong interpersonal and communication skills with the ability to build rapport with clients across varied backgrounds.

• Experience in hospice care or end-of-life counseling is highly preferred, providing insight into the unique challenges faced by clients in this setting.
• Familiarity with evidence-based therapeutic modalities applicable to grief and loss, such as cognitive-behavioral therapy or solution-focused therapy.
• A background working in remote healthcare environments is an advantage, supporting efficient virtual client engagement.
