Apple Rehab Rocky Hill is a 100-bed skilled nursing center conveniently located off the Silas Deane Highway in Rocky Hill, CT. Our facility offers private and semi-private rooms spacious enough to accommodate residents’ personal furnishings. Job Description

The Social Work Designee (SSD) supports the psychosocial needs of residents under the direction of the Director of Social Services and in accordance with state and federal regulations. The Social Work Designee assists in implementing care plans, coordinating services, and ensuring residents and families receive appropriate support throughout their stay.

Key Responsibilities:

  • Assist in developing, implementing, and documenting care plans related to residents’ social and emotional needs in the electronic medical record

  • Support admission and discharge processes, including coordination with nursing, MDS, and rehabilitation teams

  • Provide support to residents and families regarding adjustment to the facility, care planning, and discharge needs

  • Educate residents, families, and staff on residents’ rights and available services

  • Maintain and provide access to community resources and referral services

  • Assist with applications and referrals (e.g., Medicaid, community services, guardianship/conservatorship)

  • Address resident/family concerns and assist with resolution, ensuring appropriate documentation and follow-up

  • Maintain regular communication with residents, families, and interdisciplinary team members

  • Ensure compliance with all state, federal, and facility policies and regulations

Qualifications
  • Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred

  • Prior experience in a long-term care setting preferred

  • Knowledge of resident rights, admissions/discharge processes, and regulatory requirements preferred

  • Strong organizational skills with attention to detail and ability to prioritize tasks

  • Excellent interpersonal and communication skills

  • Ability to work collaboratively as part of an interdisciplinary team

  • Compassion, patience, and a positive attitude toward residents and families

  • Ability to remain calm and professional in stressful situations
