Summary/Objective
The behavioral health care manager is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.
Essential Job Duties

  1. Coordinate and support integrated behavioral and physical health care for assigned patients by collaborating closely with the medical provider, psychiatric consultant, and other treating professionals to ensure comprehensive, team-based treatment planning and follow-up.
  2. Conduct evidence-based screenings, assessments, and clinical evaluations for common mental health and substance use disorders; engage patients in care, facilitate follow-up, and assist with differential considerations within scope of practice.
  3. Provide patient education and deliver brief, evidence-based behavioral interventions (e.g., motivational interviewing, problem-solving therapy, behavioral activation), supporting patient activation, symptom improvement, and self-management.
  4. Systematically track, monitor, and document patient progress and clinical outcomes using the electronic health record (EHR) and/or registry system; monitor treatment response, medication adherence, side effects, and complications; identify and re-engage patients who are not improving or who have disengaged from care.
  5. Participate in regular caseload consultations with the psychiatric consultant and communicate treatment recommendations to the medical provider; facilitate treatment plan adjustments for patients not improving as expected, including coordination of psychotropic medication support within scope.
  6. Facilitate referrals and care transitions to specialty behavioral health, substance use treatment, social services, or community resources as clinically indicated, and develop relapse prevention and self-management plans for patients achieving treatment goals or transitioning from the caseload.

Required Education and Experience
Master's degree or higher in Social Work, Counseling, or Psychology from an accredited institution.
Minimum 1-3 years of clinical experience providing behavioral health assessment and treatment.
Required License/Certifications
Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Psychological Associate (as permitted by Texas law)
