Job description

Pay Range:

$29.55 - $44.32

A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.

Hospital Sisters Health System (HSHS) is seeking a Social Worker to join our team. Social Worker will coordinate and provide social services for patients of all ages in the patient's care setting. Social workers play an essential role in many of the aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short term counseling and therapy, and communicating patient needs and concerns to the larger medical team.
Position Specifics:
oDepartment: Care Management
oCore Function: Behavioral Health/Social Work
oSchedule: 40 hours/week, every 5th weekend
oFacility: St. Vincent's Hospital
oLocation: Green Bay, WI

Education Qualifications
  • Master's Degree in social work is required.
  • Bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.
Experience Qualifications
  • Prior experience in social work or past social service clinical experience directly transferrable to a healthcare environment is preferred.
  • 2 years of home care, hospice, discharge planning or related experience is preferred.
Certifications, Licenses and Registrations
  • Licensed as a Certified Social Worker (CSW) in the state of Wisconsin or Licensed Social Worker (LSW) in the state of Illinois is required.
  • Wisconsin: Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is preferred. Illinois: Licensed as a Licensed Clinical Social Worker (LCSW) is preferred.
  • Basic Life Support (BLS) is required.
  • In roles which travel is required, a valid driver's license, automobile insurance and own transportation is required.
