• Deliver one-on-one psychotherapy sessions to patients experiencing mental health conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct comprehensive assessments of patients’ mental health status, including their emotional, behavioral, and social functioning to inform treatment planning.
• Develop, implement, and periodically review individualized treatment plans that utilize evidence-based practices and address the unique needs of each patient.
• Facilitate family meetings to discuss treatment goals, expectations, and resources available to support patients and their families during challenging times.
• Collaborate with interdisciplinary teams, including physicians, nurses, and spiritual care providers, to ensure coordinated and comprehensive patient care.

The Licensed Clinical Social Worker will primarily serve hospice patients, focusing on those dealing with terminal illnesses and their families. You will work with a diverse population from various backgrounds, requiring an understanding and sensitivity to different cultural dynamics and individual experiences in the face of loss and grief.

• Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrable communication and interpersonal skills, necessary for establishing therapeutic rapport with patients and productive collaboration with healthcare professionals.
• Solid clinical skills with the ability to conduct thorough assessments, analyze patient needs, and design impactful treatment plans.
• Proficiency in utilizing electronic health records (EHRs) and other digital health software for effective documentation and patient care management.

Experience within hospice or palliative care settings is preferred, as it equips candidates with the unique skills needed to address the specific emotional and mental health challenges faced by terminally ill patients and their families. Familiarity with grief counseling and end-of-life care practices is also advantageous.
